Your First Visit to Seattle Periodontics and Implant Dentistry

When making your appointment we will ask you to provide your dental and medical insurance information so that we can complete a complimentary benefits check before you arrive. We will also ask you some questions regarding the reason for referral to our practice and your current oral and medical health. Please remember to bring your dental and medical insurance cards with you as well as a list of any medications you are currently taking to this appointment.

Your first visit to Seattle Periodontics and Implant Dentistry will involve a few special steps so that we can get to know you. On arrival you will be seated in an exam room and a dental assistant will confirm with you that we have all the relevant information. We will ask about the medications or supplements you are currently taking. We will review the current radiographs / x-rays received from your referring dentist. Sometimes additional x-rays are needed to assist us in the diagnosis and treatment planning of your specific dental needs and will be taken at this appointment. Your periodontist will complete a thorough periodontal examination. You will get a detailed explanation of the current status of your oral health so that we can partner effectively to meet your dental needs and expectations. Treatment options will be discussed with your general dentist and communicated to you at this visit or a subsequent consultation as needed.

You can then meet with our Treatment Coordinator and will get detailed information regarding your personalized course of treatment. Associated costs and insurance coverage will be discussed with you. This discussion would take into account any dental or medical insurance that could be used to assist in payment for treatment, your concerns about scheduling treatment to accommodate your current or future financial situation and payment options available to you. We work on behalf of our patients, not an insurance company and our recommendations are based on what is necessary to optimize your oral health, regardless of what your insurance company may pay for or allow. We understand that specialized dentistry treatments are not typically "budgeted" for and you may need to look at financing options that we facilitate for you through services such as CareCredit. Our main objective is to be able to meet your periodontal needs or place your dental implant(s) effectively, at a reasonable cost and with measurably predictable and satisfying results.

Dental Insurance accepted at our office: Our office is a Premier Provider in the Delta Dental network. In addition we accept all standard indemnity dental insurance (i.e. Cigna, Aetna, United HealthCare, Metlife, etc.) and will file the claim(s) for you as a courtesy. We do not accept or participate in any dental/health maintenance organization plans.

We will attempt to contact your insurance company prior to providing services, however, information gathered from your insurance company is considered an estimate of benefits and not a guarantee. Most insurance policies have specific limits or restrictions for dental care and you must contact your insurer directly for any questions regarding your coverage.

As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that you provide us with the most current information with regard to your insurance coverage.

Co-Payment
Please be aware that insurance is intended to reduce the cost of dental care and often does not cover the total fee. You will be responsible for the portion that is not covered by your insurance. Payment is due on the day services are rendered.

For your convenience, we accept Visa, Discover, American Express and MasterCard. Payment Plans Although we do not offer "in house" financing, we do work with an outside financing company that may meet your needs. It is called CareCredit. CareCredit offers a healthcare credit card exclusively used to pay for health care.

The Health Insurance Portability and Accountability Act of 1996 instructed healthcare providers of the necessity to ensure that the patient's personal and private information is secured and kept private. We have in place, numerous controls to safeguard and keep private your personal information. Ultimately you have control of our use of your protected information as dictated by the HIPPA act.
